Canada is in the midst of a demographic shift, with more people over 65 than under 15 for the first time in history. By 2036, one in four citizens will be over 65.
This aging population strains hospitals, with many beds occupied by seniors awaiting long-term care (LTC). More than 45,000 people in Ontario are on LTC waitlists.
Privately run homes are often unaffordable, and current care models can’t keep pace. Dementia and other brain disorders add further challenges, causing mental, physical, and financial stress for patients and caregivers.

The Founding Partners are supporting Baycrest@Home 2.0, an innovative and accessible technology platform that is being developed to disseminate impactful healthcare and services early in the healthcare journey. B@H will support people who are aging at home, which will help prevent crises and hospital visits.
Evidence shows that filling four major gaps in current home and community care services can impact the ability to live independently:
Baycrest@Home will curate a mix of Baycrest-original and third-party products and services, organized along three main themes:
Safe@Home: Smart-home enhancements that increase the safety and independence of older adults, and their connection with caregivers;
Help@Home: Practical services and information for caregivers to help them deliver and coordinate care, and deal with the emotional stress of providing care;
Health@Home: Virtual and face-to-face clinical services offered in partnership with health providers that improve the health and quality of life of both older adults and their caregivers.
Who are the Founding Partners?
We’re not just imagining a better way to support older adults at home – we’re creating it.
In 2019, a visionary group of Founding Partners stepped forward to fund and help launch Baycrest@Home 1.0, a groundbreaking program bringing Baycrest’s trusted care and expertise into the homes of older adults and their families.
Their commitment – $18,000 over three years ($500/month for 36 months) – has been instrumental in powering B@H.
